This form is required for all person(s) who are requesting a Letter of Good Conduct from the Ridgewood Police Department.  This letter is often requested as part of an application process and is only provided after a thorough internal criminal records check.  The letter speaks to incidents / police involvement within the Village of Ridgewood ONLY, and is not a representation of an applicant’s actual criminal history.  The letter, also, does not address Motor Vehicle History (driver’s history), outside of violations of quasi-criminal motor vehicle offenses (e.g. DWI).

Once the form has been submitted to the Record’s Bureau the letter is typically made available between five (5) and ten (10) business days.

Please note: This form must be submitted by the person listed as the subject of the request.  Forms submitted on behalf of, or as a means of a background investigation on a subject, will not be accepted.

If you have any questions regarding this form, please contact the Records Bureau.
